LETTER OF AUTHORIZATION – TERMS & CONDITIONS

I, the undersigned, hereby authorize AssuredLink ("the Company") to act as my agent for the purpose of porting my telephone number(s) from my current service provider to AssuredLink's network.


By signing this Letter of Authorization (LOA), I confirm that:


1. I am the account holder of record for the telephone number(s) listed in this application, or I am an authorized representative with authority to act on behalf of the account holder.


2. I authorize AssuredLink to submit a porting request to my current carrier on my behalf — including US carriers (AT&T, Verizon, T-Mobile, Comcast, etc.) and Canadian carriers (Bell, Rogers, Telus, etc.) — and to provide any necessary information to facilitate the transfer.


3. I understand that the porting process may take 3–10 business days, and that my current service with my existing provider will be cancelled upon successful completion of the port.


4. I acknowledge that one-time setup fees and number porting charges apply, as outlined in AssuredLink's current pricing schedule.


5. I agree to AssuredLink's Terms of Service and Privacy Policy, and I consent to being contacted by AssuredLink regarding my service.


6. I understand that providing false or inaccurate information on this form may result in delays or cancellation of my porting request.
